South Denbighshire Community Partnership (SDCP) supported by Citizens Advice Denbighshire (CAD) are pleased to announce that they have been awarded a grant of £349,847 over a four-year period by the Big Lottery Rural Programme.

 

The grant will enable South Denbighshire Community Partnership & Citizens Advice Denbighshire to work together with the Communities of Corwen, Carrog, Glyndyfrdwy, Llandrillo, Cynwyd, Melin y Wig, Betws Gwerfil Goch, Gwyddelwern and Bryneglwys to reduce isolation, improve wellbeing and increase resilience primarily for older people and those unable to access services.

 

Specifically this will support existing and develop new social activities and events, improving access to welfare benefits and grants, extending our community transport  to include a dial a ride and meal delivery service, whilst at the same time providing opportunities for local people to learn new skills and be involved in the design, running and delivery of services.

Ican Mental Health Services

In April 2021 SDCP were commissioned by Betsi Cadwaladr to be an ICAN Centre.

 

We offer a weekly programme of regular events and activities all of which address mild to moderate poor mental health. Some examples include:

  • Lifestyle coaching delivered by Advance Brighter Futures

  • Crafty Social Club

  • Buddy Callers

  • MIND group session

  • Men’s Meet and Make

  • In centre Digi lab providing access to online mental health resources

  • Special events- past activities have included bushcraft sessions, pamper and self-care and family circuit

 

Referral

Referrals can be made by health professionals and support agencies by emailing enquiries@sdcp.org.

 

Self-Referral

However, you do not need a referral. We operate an open-door policy where people can walk in and speak to a member of staff who will discuss what is available and arrange appointments with relevant support agencies and/or introductions to existing groups in the Centre.

Cawl a Chan

Cawl a Chan is a social group for people in the South Denbighshire area to come and spend time with people. There is a cost of £1 per person which includes entry, soup and bread, cakes and tea and coffee, as well as brilliant company or why not enjoy a lovely hot, one or two course homemade meal in partnership with our recently launched "Meals On Wheels" service.  

 

This is followed by activities of your choosing. Some play dominos, some play board games, some sing, some just talk and sometimes we have guest speakers and performers in to give talks and presentations. Everyone is welcome to join in with this group and we even have transport available for anyone who has difficulties accessing the public transport network.

Cawl a Chan is on every Tuesday afternoon from 12:30-3:00.

Edeyrnion & Dee Valley
Meals on Wheels Service

We deliver hot, nutritious meals within the Dee Valley area to residents who are elderly, shielding, have difficulty with mobility, recovering from an accident or illness. Your meals on wheels delivery driver will happily deliver to your door Monday to Friday on your preferred days of delivery. Meals are delivered hot and ready to eat between 12 pm and 2:30 pm. We can’t give exact delivery times, but we will deliver at a similar time daily.

We are very proud of both the quality and variety of our balanced, nutritious meals, which adhere to strict catering guidance. We have a wide selection of meals suitable for dietary, ethnic, and religious choices. Our menu repeats every 4 weeks and refreshes in Winter and Summer. Prices are £6.50 for a meal & dessert, with the option of adding Soup and a Sandwich for an additional £4. Payment can be made in cash or securely over the phone.

Corwen, Cynwyd, Llandrillo, Carrog, Glyndyfrdwy, Bryneglwys,  Bettws G.G, Melin Y Wig Gwyddelwern, Llangollen, Pengwern, Llantysilio, Pentredwr,  Eglwyseg.

Cogog Meal Delivery service

Cogog is an exciting new project funded by BCUHB and managed by Denbighshire County Council which is initially to be piloted in South Denbighshire.

Cogog will provide access to affordable food for all, sourced by your local community whilst at the same time reducing food waste. Cogog is working with Well Fed to deliver these meals.

We know cooking from fresh can sometimes be time-consuming, which is why we’ve sourced a range of tasty ready meals, so you can enjoy the goodness of a fresh home-cooked meal straight from your oven or microwave.

The first phase of the project will provide healthy nutritious and balanced ready meals that can be ordered over the phone and delivered to your door, or through one of our distribution points.

The food is sourced direct from farms or where possible, local producers. This means the food is as fresh as it can be, and we also reduce the food mile footprint.

When alternatives become available, we replace products that cannot be recycled and if this is not possible, source only products that can be re-used multiple times.

Edeyrnion & Dee Valley Dial-A-Ride Scheme

Telephone SDCP and book the vehicle for Doctors Appointments, Hair Appointments, Shopping Trips or simply to visit friends. The vehicle will pick you up at the door, take you where you want to go within the Edeyrnion Region and return at an agreed time to take you home. It is as easy as that!


Membership is just £15 per person per year and a small charge is charged for each journey taken.
